licensed practical nursing certificate requirements

Admission requirements

All students interested in the LPN program must submit the following:

  • The programmatic LPN application
  • Proof of high school diploma, or GED
  • An ATI TEAS Exam (with a minimum score of 48%) in order to be eligible for the admissions process
    • You can take the TEAS test on-site at Goodwin University free of charge
    • If testing outside of Goodwin, TEAs scores must be sent directly to Goodwin University via the ATI website: please select “Goodwin U LPN”
    • Only ONE SCORE per application cycle is accepted: Submit the score that you already have OR retake the exam if needed
  • Complete FAFSA and meet with a financial aid counselor to accept your awards
    • Enrollment into the program is not complete until you meet with one of our counselors to sign off to accept your aid
    • Students who are not utilizing financial aid must meet with our Business Office to set up a payment plan

Current students will work with their academic advisor to submit requirements. Student who are new to Goodwin, or need to re-enroll, will submit their requirements through the Admissions department

Please be aware that the LPN department will collect the following materials from students prior to beginning the program:

  • Physical exam from a health care provider: This report will describe the student’s physical and emotional health prior to starting the program. Submit your physical exam completed and signed by a medical professional in the past 12 months. Physical exam MUST include physician stamp OR contain a physician provided DEA number. Only required upon entrance to the program.
  • Immunizations: All required immunizations must be completed before the start of the first nursing course
    • MMR (Measles, mumps & rubella)
    • Varicella (Chicken pox)
    • Hepatitis B: the Hepatitis B vaccine series which may be in progress. If a student elects not to have the Hepatitis B vaccine series, a signed declination form must be in his/her file.
    • Tuberculosis (TB)
    • Tetanus, Diphtheria & Pertussis (TDaP)
  • Influenza vaccination: All students in the program must receive a flu vaccine annually
  • COVID-19 and Booster: Submit documentation of your COVID-19 vaccination(s) (1st and 2nd). Documentation must include the vaccine manufacturer and Lot#
    • If your series is in process, submit your first vaccine and a new alert will be created for you to provide your second vaccination, if applicable
  • Cardiopulmonary Resuscitation (CPR) certification: The only 2 courses accepted are Healthcare Provider BLS (American Heart) and Professional Rescuer BLS (American Red Cross).
  • Background check/drug screen: All students accepted to the LPN program must submit to a background check and drug screening. Goodwin University covers the cost through the Navigators Scholarship for LPN certificate. However, if the drug screen result is “negative dilute,’ the student must assume the cost of a second drug screen. A positive drug screen, not substantiated by a healthcare provider, will prevent the student from entering the program. Students will be notified if the background check reveals any criminal record that might interfere with future licensure. If a clinical facility prohibits the student from participating in a clinical experience based on this result, the department will make two additional attempts to place the student in an alternative facility. If neither of the two further attempts yields a placement for the student, (s)he will not be permitted to progress with the program.

Any questions regarding documentation should be referred to the clinical coordinator Sue Jahn at .
